Specialized Services:

Home Safety Evaluations: Independent Living Therapy Services will evaluate an individual’s needs and provide recommendations including environmental modifications, adaptive equipment or occupational therapy intervention to maximize safety and improve performance with daily tasks.

Home safety evaluations are often beneficial for those who:

• Have experienced a change in their medical condition limiting their ability to complete daily tasks
• Have experienced repetitive falls
• May not be safe in their home environment and
• Now require the use of a mobility device such as a cane, walker or wheelchair.
• Individuals who would like to remain at home or explore alternatives to assisted living.

Wheelchair Evaluations: Independent Living Therapy Services is dedicated to helping individuals obtain an individualized prescription for wheeled mobility.  This evaluation will be completed by a RESNA certified Assistive Technology Practitioner who will focus on improved functional performance and specific client needs to determine specific equipment recommendations.  We work closely with your physician and local durable medical vendor of your choice throughout the process to ensure timely evaluation and service delivery.  A wheelchair evaluation is often beneficial for those who:

• Have a neurological diagnosis which impacts mobility
• Are experiencing difficulty getting from room to room within their home
• Are experiencing repetitive falls
• Currently have a mobility device that is not functional or usable in the current environment

A physician referral for therapy services is required.
